WHAT TO EXPECT
1.
Fill out a from by going to the appointments page and we will get back with appointment time.
2.
On the day of your appointment, arrive at
2404 S. Tyler St.
3.
An appointment for laboratory services will be scheduled prior to clinic appointment. You must keep lab appointment or clinic appointment will be cancelled
4.
Arrive promptly. Due to the nature of the clinic you may have to wait to be seen; all staff are volunteers. Patients will be seen by appointment only. Follow all instructions provided with your appointment notice such as bring all meds with you to your appointment.
5.
If you cannot keep your appointment you must cancel.
No shows will not be rescheduled.
